How to Enter Your Prescription
Enter the SPH, CYL and AXIS values exactly as they appear on your eyeglass prescription. If your prescription does not contain CYL or AXIS values, leave the corresponding astigmatism fields at the appropriate zero or blank setting shown in the order form.
If your prescription provides a PD value, enter it in the PD field. If you have separate right and left PD measurements, select the separate PD option and enter each value individually.
